Case Manager provide individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnessesServices include advising family caregiversProvide patients with information and counseling, and make referrals for other servicesReports To the Supervisor of the UnitDuties and
Responsibilities:
Performs admission, concurrent, and retrospective reviews on all patientsEnsures that status and level of care determination are accurate upon admissionEnsures that pre certifications/authorizations are obtained and documented appropriately for all admissions, surgeries, and proceduresFollows Medicare guidelines regarding utilization reviewSubmits telephonic and/or fax notification/clinical informationFollows up with insurer for authorizationsIdentifies potentially high risk patients and refer to Social WorkProvides effective clinical documentation and conducts Initial Assessments to develop plan of careEnsures that treatment plan is implemented, re-evaluated, and updated dailyFacilitates effective communication between members of the treatment teamCoordinates meetings w/ families and treatment teamFacilitates arrangement of post-acute care servicesActs as an advocate for the patient/familyFollows Medicare guidelines regarding patient choice, admit/discharge notifications, and appealsMaintains accountability for facilitating clinical patient progression through a defined plan of care to achieve optimal outcomesIdea Provide individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnessesServices include advising family caregiversProvide patients with information and counseling, and make referrals for other servicesIdentifies, document, and report quality, cost, and risk variances related to patient's plan of careEducation & Experience Requirements:
